Text for the book cover of free publishing option with ISBN and copyright page

Sorry if this has been discussed previously, but could I ask a question on the text for the book cover, so that I can make 100 % sure that I follow everything correctly

What text must be included on the -
- front cover
- spine
- back cover.

Obviously the title of book and authors name and the space on the back for barcode etc...

Where do I put the bit about youwriteon.com as publisher? I am including the name of book cover illustrator on title page, but can I also include it on front cover?

I have been through many books and some have publisher on front cover and some don't etc...

Also - with copyright page where it says
copyright (C) text ...(name of author)
with the cover being illustrated for me do I include that underneath as (C) cover design ...(name of illustrator)

If it is their copyright then yes, include the (C) cover design underneath you as author.
